Ethics approval and consent to participate

: The study was conducted in compliance with the Helsinki Declaration to protect human subjects and was approved by the Medical Ethics Committee of Medical School of Nanjing University (the ethics approval number provided by the board was 2,011,052). The written informed consent was obtained from every patient included in the study. If the patients were less than 18 years, the consents would be acquired from their parents.
