Ethics approval and consent to participate
: The trial was approved by the National Research Ethics Service Committee North West (UK) Central (REC reference: 15/NW/0546) and by the MHRA (EudraCT number: 2014–003412-37). Written informed consent is provided by all patients prior to any trial specific procedures. The trial is overseen by externally appointed Trial Steering and Data Monitoring Committees. All protocol modifications are submitted to the REC/MHRA and local investigators. The trial is also registered with Current Controlled Trials ISRCTN50550256 (assigned 28/07/2015).All participants will be informed of the results personally by post. The results of this study will be reported to the trial steering committee, data monitoring committee and our funder, published in relevant high-quality peer-reviewed journals and presented at national and international conferences.
