: The study is approved by the Regional Committees for Medical and Health Research Ethics (REC) (2017/613). It is registered at ClinicalTrials.gov (Registration no. NCT03580213). The patients who agree to participate in the study will sign an informed consent form. Important protocol amendments will be discussed in the project group and conferred with REC before finally informing both REC and ClinicalTrials.gov.
