Ethics approval and consent to participate
: The interviewer-based-survey with individual-linked information to registries was approved by the Danish Data Protection Agency (J.nr. 2015-54-0999). All participants were informed about the aim of the study and were assured that participation was voluntary, and results would be anonymised. Thus, upon participation the respondent provided consent to participate in the survey. This type of study does not require further formal ethical approval, according to Danish law.
