Ethics approval and consent to participate

: Informed, written consent was received from all included patients, ESPs and OSs. The study was approved by the Danish Data Protection Agency (ref. no: 1–16–02-724-17) and the management at the Hospital. The study was preregistered at(NCT03343951). The Ethics Committee of Central Denmark Region accepted initiation of the study and reviewed the study as non-notifiable (inquiry: 8–1–45-70-10-17). According to Danish law no further approvals were required.
