Ethics approval and consent to participate
: Our retrospective study was exempted from Ethics Committee approval after review by the Medical Research Ethics Committee of the South-Eastern Norway Regional Health Authority due to the fact completely anonymized data was used. Data were collected, stored and analyzed in strict compliance with data protection and athlete confidentiality. Informed consent was waived because all our epidemiological data was anonymized and unidentifiable. An institutional review board (IRB) approval was obtained from Boston University (#H-36593).
