: The NZJR has Institutional Review Board IRB approval for publication of results stored in its registry (Canterbury Ethical Committee 1998). Therefore no formal IRB approval was required for this study. Patients contributing their data to the NZJR provide formal written consent for this and for the analysis of their data and subsequent publication. The New Zealand Joint Registry is the property of the New Zealand Orthopaedic Association i.e. it is closed to the public.
