: Ethics approval and processes for informed consent were obtained from the VA Puget Sound Health Care System Institutional Review Board (01860) and the University of Washington Institutional Review Board (STUDY00010918). There are separate informed consent processes at each institution for the components of the project conducted at VAPSHCS, and the components of the project conducted at UW. At VAPSHCS, at the start of the study, all informed consent was written informed consent. Subsequently, Institutional Review Board approval was obtained to provide an Information Statement to potential participants and a summary of both VAPSHCS and UW components of the project. The Information Statement is verbally reviewed with the potential participant, and verbal informed consent is obtained. At UW, an electronic Information Statement is provided to potential participants, and participants indicate their acknowledgement of the Information Statement and consent to participate in the study electronically.
