: The study followed the principles of the Declaration of Helsinki and its later amendments. It was approved by the Northern Ostrobothnia Hospital’s District Ethical Committee 94/2011 (12.12.2011). Participation in the NFBC1966 was voluntary and each study participant granted their written informed consent. Information on personal identity was encrypted and pseudonymized before being handed over to the researchers.
