: This study was approved by the Fife & Forth Valley Research Ethics Committee. REC reference number: 09/S0501/1.A study information leaflet will be given to each potential participant to inform them of the anticipated risks and benefits of taking part in the study. Informed signed consent forms will be obtained from the participants in all centres, by an individual who is trained in GCP. Patients will be given sufficient time to accept or decline involvement and are free to withdraw from the study at any time.
