Ethics approval and consent to participate
: The study was approved by the medical ethical committee of the Vrije Universiteit Medical Centre (2014.585 [A2016.155]). All patients received a patient information letter, explaining that all data was treated anonymously, that they were free in their choice to participate, and that refusal would not influence the care they would receive during their intake appointment. All measurements were considered non-invasive and were – besides the SROH questionnaire and saliva sampling – already part of the regular intake appointment. Therefore, verbal consent was obtained from all participating study participants at the time of recruitment, as approved by the medical ethical committee.
