: This was a register-based study using anonymized data. According to Danish law (The Act on Processing of Personal Data: Act No. 429 of May 31, 2000 with amendments), approval from an ethical committee and informed consent from the study subjects are not required when performing register-based studies [Thygesen et al. 2011*]. Register-based studies should, however, be approved by The Danish Data Protection Agency. Accordingly, approval for the study was obtained from the Danish Data Protection Agency (approval no. j-no. 2015–41-3790).The study did not involve contacts with participants or human material.
