: The study was approved by the institutional ethics committee of Tehran university of medical science (NO: IR.TUMS.DENTISTRY.REC.1400.138) and complied with the declaration of Helsinki and good clinical practice guidelines.In the beginning the questionnaire we explain to the participants that: “Completing the questionnaire by you as is considered as you are inform consent to participate in this study”.The people who had received fluoride varnish therapy service in PSOHPP were school pupils in the age group of 7 to 14 years, and before providing the service, written consent had been obtained from their parents or legal guardians.
