: The Research Ethics Committees in the Mater Misericordiae University Hospital Ethics Committee (Reference number: 1/378/1685) and Tallaght University Hospital/ St. James’s Hospital Joint Research Ethics Committee (REC Reference: 2014–12 Chairman’s Action 8) granted full ethical approval for the study. Respondents were informed that return of the questionnaire was viewed as consent to participate. Any identifiable data were removed from the dataset during analysis.
