: All patients gave written informed consent. Dutch law specifies that ethics approval is only needed when ‘participants are subject to procedures or are required to follow rules of behaviour’ (). As this was not the case, the approval of the local research ethics committee (REC AMC Amsterdam) was waived, as confirmed in writing.
