Ethics approval and consent to participate
: The study was approved by East of England Cambridge Central Research Ethics Committee Reference 16/EE/0330. Written informed consent was obtained from all interviewed patients. Nine patients from the cohort had deceased before consent could be taken to review their medical notes; as agreed with the ethics committee, data for these women were collected by a member of the medical team and individual patient data anonymised before being passed on to the study researchers. Following the agreement of the ethics committee, the research and development department for Christie NHS Trust granted administrative permissions to review patients’ medical notes.
