Ethics approval and consent to participate
: University of Southampton School of Health Sciences ethical approval (submission 30,766; 27/03/2018) and Health Research Authority (HRA) governance approval (IRAS: 231837; 26/06/2018) were gained. A participant information sheet accompanied distribution of the survey link, including details of the length of the survey, data storage, the investigator and the purpose of the study. Participants provided consent via a tick box at the start of the questionnaire.
