Ethics approval and consent to participate
: Ethical clearance was obtained for the Swiss study by the Bernese Cantonal Ethical Commission and participants provided written informed consent to participate (KEK 2016–00896). The Delphi study and consensus meeting were authorised by the Bernese Cantonal Ethical Commission with a declaration of no objection, meaning that they are exempt from further review (req-2019-00200). For the Delphi study, participants will be informed that participation implies consent to participate, and for the consensus meeting, attendance implies consent to participate. Participant consent will be obtained for the Stage 2 studies as part of independent research protocols.
