: This study was conducted after obtaining approval from the human research ethics committee of Chiba University (Approval Number: 29-64; November 1, 2017).Together with the survey form, the participants received a document that provided information on the following: the purpose and the method of this study, respect for participants’ free will, protection of anonymity, and data management. The study invitation letter stated these explanations for all the participants. We also explained that the participants would be deemed as having given consent to participate upon returning a completed form. With this procedure, informed consent was obtained from all the participants.We carried out this study in accordance with the Declaration of Helsinki as well as an approved method by an ethics committee of Chiba University.
