: All methods were carried out in accordance with relevant guidelines and regulations including the ethical principles for medical research involving human subjects as outlined in the Declaration of Helsinki, World Medical Association. Consent to participate was obtained at every stage of the survey instruments development. Participants were provided with both written and oral information in advance of focus group and cognitive interviews. Participants provided oral and written informed consent to take part. In view of the potentially emotive nature of the subject, all participants received information on bereavement supports and participants were aware of their right to withdraw from the study at any point in time.Data protection procedures were observed and assurances of confidentially were provided. Correspondence between the lead author (DÓC) and participants took place via HIQA email, and all emails containing personal identifiable information were deleted. Any identifiable data were removed from the dataset during analysis thus ensuring no participants were identified by comments made during any element of the survey’s development.The authors submitted an application to the Royal College of Physicians in Ireland (RCPI) Research Ethics Committee on behalf of the National Care Experience Programme to undertake the National End of Life Survey. Ethical approval for the study was obtained from the Royal College of Physicians of Ireland Research Ethics Committee with the National End of Life survey being undertaken in 2022.
