: The study was approved by the medical faculty’s ethics commission of Ludwig-Maximilians-University Munich (Az 558–15). The interviews were conducted after clarification and written Informed consent of the participants. In the beginning of the interviews, interviewees received information about the aim of the study and signed the informed consent form for participation. Confidentiality and voluntarily withdrawal from the study were approved by the study conductors. All methods were conducted in accordance with the ethical standards of the declaration of Helsinki (1964, last revised in 2000) of the World Medical Association.
