: Regarding the MetroSud cohort, the study was approved by the Bellvitge Hospital Research Ethics Committee with medicines (CREm) in accordance with Spanish legislation and was performed in accordance with the Helsinki Declaration of 1964. The need for patient informed consent was waived by the ethics committee. Bellvitge's CREm decision was the basis for the approval of the remaining hospital centres.Regarding the Leeds Teaching Hospitals cohort, approval was given following institutional governance review following submission to the Clinical Audit Database at Leeds Teaching Hospitals NHS Trust. In view of the retrospective nature, individual patient consent was waived as appropriate data protection safeguards were in place.
