: This study was conducted in accordance with the Declaration of Helsinki and received a positive ethics approval from the Ethics Committee of the Medical Faculty, the institutional review board (IRB) at Heinrich Heine University Duesseldorf, [reference 2021–1600, August 2021] and all participating centers (Supplementary Table 1). All data obtained were collected in routine clinical care, extracted from electronic hospital databases and strictly de-identified.
