: The West Virginia University Human Research Protection Program (HRPP) Institutional Review Board (IRB) approved this study (IRB number: 2408021376). The Principal Investigator (PI) and trained personnel, including faith community nurses, will recruit subjects. Patients with advanced heart failure (HF) and their caregivers will be asked for permission to be contacted, the study will be explained, and written informed consent will be provided. This study aimes to determine whether the intervention helps manage worsening HF symptoms at home and facilitates the signing of advanced directive forms. The faith community nurse interventionists have been trained to conduct home visits and follow-up calls. These nurse interventionists will train local volunteers to assist with the experimental group home visits and telephone calls. Consent forms will describe the study, group assignments, timelines, potential benefits and risks, financial considerations, and confidentiality measures. Standard care will not be altered for any participant.
