: The primary study and conduct of any secondary study, subject to supervision by the primary research team, was approved by the King’s College Hospital Research Ethics Committee (ref no.: 09/H0808/85). This study was conducted on a de-identified dataset. No further new data was gathered from participants for the purposes of the current study. The storage, handling, and supervised secondary analysis of QUALYCARE, in addition to primary approval, complied with the QUALYCARE Study Confidentiality Agreement, and the ethical approval of the original study.In the primary study, together with the invitation letter and questionnaire, potential participants were mailed the study information sheet, a reply slip to decline participation, and a bereavement information leaflet.
