: A written informed consent was obtained from all the participants, and data security and anonymity were managed in accordance with data protection regulations. The project was approved by the Data Protection Officer at Akershus University Hospital. Based on a remit assessment form the Regional Committee for Medical and Health Research Ethics South-Eastern Norway evaluated the project as health services research not requiring ethical approval (reference number 687379).
