Ethics approval and consent to participate
: The study has been registered with the Registry for International Development Impact Evaluations (RIDIE-STUDY-ID-58261b2f46876) and Indian Council of Medical Research (ICMR) National Clinical Trials Registry of India (CTRI/2016/11/007482) []. Ethical approval was obtained from the Institutional Ethics Committees of All India Institute of Medical Sciences (AIIMS) Bihar, Odisha and Chhattisgarh. Verbal consent as approved by the ethical committee was obtained from parents/guardians of each respondent and also from the respondent before administering the questionnaire. As the study implementation was done in rural parts of India, where the selected districts are mostly inhabited by illiterate population, only verbal consent was taken into account. Utmost confidentiality of information and anonymity of respondents was ensured.
