Ethics approval and consent to participate

: Ethical approval was granted by a National Health Service (NHS) Ethics Committee (18/NW/0007) and the Health Research Authority, England. Local Research and Development permissions were granted by agreed NHS trusts. Participants who expressed an interest were sent information packs that included a participant information sheet and a consent form. Written informed consent was sought from all participants prior to the interview.
