: Our research was approved by the University of Utah’s Institutional Review Board on 08/08/2013 (IRB #65794). Participants who formally enrolled in the survey arm of the study provided written informed consent. Those under 18 were not eligible to enroll in the survey portion of the study and thus parental consent is not applicable.
