: Ethical approval was obtained from the University of KwaZulu-Natal Humanities and Social Sciences Research Ethics Committee (HSSREC) (HSS/0319/018), KZN Department of Health (HRKM235/18) and the World Health Organisation Ethics Review Committee (ERC 0003101). All participants provided written informed consent for the series of IDIs. Additional informed consent was obtained for the photovoice activity, including a signed release allowing the photographs to be used for study purposes. For all data collection activities, participants received a small payment (SAR150, approx. USD8) to compensate for their time. In addition, participants in the photovoice activity received refreshments.
