The study protocol was approved by the Ethics Committee of the Helsinki and Uusimaa Hospital District (Dnro 457/E8/04 and 343/13/03/03/2015) and was registered in the ClinicalTrials (NCT00744172). The use of data from the Care Register was authorized by the national health authorities, and all data were used in accordance with national and institutional regulations.
