Ethics approval and consent to participate
: Ethical clearance and approval to conduct this research were obtained from the Research and Ethical Review Committee of the Department of Nursing and Midwifery, School of Allied Health Sciences, College of Health Sciences, Addis Ababa University (Approval no.152/2017, AAUMF02–008). During the household survey, after explaining the objective of the study to all participants, verbal consents to provide information were obtained as the respondents unable to read and write. Regarding data collected at district and Kebele levels, permissions were requested and obtained from District Health and Kebele Administration Offices. Both were approved by the Research and Ethical Review Committee of Addis Ababa University.
