: This study was approved by the Institutional Ethics Committee of the Faculty of Nursing, University of Indonesia, and official permission was obtained from Dr. Cipto Mangunkusumo National Hospital (letter number: 315/UKA/I/IV/2011). The study procedures followed the guidelines of the Helsinki Declaration. Participation in the survey was contingent voluntarily, and as a result, no personal data of the patients were collected, thus preserving their confidentiality and privacy. After informing the children and their families of the purpose of the research and the objectives for which the data would be used, each participant provided verbal and written informed consent. For children under the age of seven, their parents’ written informed consent was obtained. Both children and their parents’ written informed consent was obtained for children aged 7–18. The participants’ anonymity and confidentiality were guaranteed.
