Ethics approval and consent to participate
: Participants were provided with an information sheet describing the objectives of the study. Participation was voluntary and participants reserved the right to withdraw from the study at any point without any consequences. Implied consent was obtained from all participants by completing and returning the baseline questionnaire. Participants were compensated for their travel and accommodation expenses. Permission to conduct the study was obtained from the Mzimba District Health Authority. Exemption from ethical approval was granted from the Malawi National Health Science Research Council as it was a programme audit and the aim of the research was to inform district policy.
