Ethics approval and consent to participate

: Ethics approval was granted by the University of Sydney Human Research Ethics Committee, research protocol number 2013/1006.Before the online survey participants were informed, through a written consent form, about the study’s goals, their voluntary participation and that by participating they consented to the use of their data for research purposes. We also emphasised that although identifiable data was collected data would be anonymised at the beginning of the analysis.
