: Institutional review board (IRB) approval was obtained for all phases of this longitudinal study. Study years 1, 3, and 11 received expedited review and approval by the Memorial Hospital of Rhode Island IRB, home of the Brown family medicine residency. Study-year 20 received expedited review and approval by the Brown University IRB. Both of these Institutional Review Boards (one at a teaching hospital and one at a university) ensure that all approved studies comply with USA national guidelines and the Declaration of Helsinki.
