: This study constituted a quality improvement project and did not use any personal health information or other sensitive personal data. According to Swedish law, studies not involving sensitive personal data are exempt from the need for ethics approval (Ethical Review Act, 2003:460) and hence formal informed consent was waived. However, all trainees consented to participate and received information about the study when applying to the training program. The study adhered to the Declaration of Helsinki.
