Ethics approval and consent to participate: In Demark no Ethics Committee reviews studies that do not include biological material (‘The Danish Act on Research Ethics Review of Health Research Projects’). The Ethical Committee of the Capital Region of Copenhagen was asked in a letter ref nr 63795 and confirmed that a review an ethical review was not necessary. However, we nevertheless adhered to ethical principles (e.g., voluntary participation, informed consent, anonymity, confidentiality, avoiding harm and free from scientific misconduct) as, for example, outlined in the Declaration of Helsinki. According to the Danish Act on Research Ethics Review of Health Research Projects participants consent is not needed in a study like this. Participants written approval was collected for alle interviews.
