: The study received ethics approval from the University of KwaZulu-Natal Humanities and Social Sciences Research Ethics Committee (ref HSS/1109/017D). Written informed consent was obtained from all participants before they took part in the study. For adolescents aged less than 18 years, we obtained written consent from their older carers and written assent from the young person themselves.
