: The AHHA trial was reviewed and received a favourable ethical opinion from LSHTM reference: 16181 and by the College of Medicine Research and Ethics Committee (CoMREC) reference number P.11/18/2539). This empirical work was reviewed and approved by COMREC and its reference number is P08/17/1233. We sought institutional support from all participating institutions and their letters of support were submitted to CoMREC. Individual written consent was obtained from all research participants who were interviewed through observations, FGDs, and IDIs. Informed Consent included permission to have the results of the study disseminated in research dissemination conferences and publication in peer-reviewed journals. The methods of this study were carried out following relevant regulations and guidelines e.g. the Declaration of Helsinki and research guidelines for Malawi.

: All participants voluntarily agreed to have their views disseminated through publication and also presented at local and international conferences and meetings.
