: Ethical approval for the study was obtained from the University of Gondar, College of Medicine and Health Sciences, Ethical Review committee (Ref No IPH/1980/2013). An official letter from Public Health Institute was written to the Central Gondar Zone health department to get permission to carry out the study. After the purpose, benefit, risk and method of the study have been explained, informed consent was taken from each participant. The respondents participated voluntarily and refusal to respond to some questions or withdrawal from the study was possible at any time if needed. The written consent form was attached to each questionnaire and the participants read and give their consent before the actual data collection. Personal identification in the questionnaire was removed to protect the privacy and confidentiality of the respondents. All the methods in this research were conducted according to the Helsinki Declaration [CitationRef removed].
