: The protocol for the research project has been approved by the Norwegian Social Science Data Services where aspects of privacy protection were assessed (Approval September 17, 2012, project number 31360). All methods were carried out in accordance with relevant guidelines and regulations. An informed consent for participation and publication of the results was obtained from all subjects and/or their legal guardian(s).
