: The Kilifi County Research Governance office, the KEMRI Scientific and Ethics Review Unit (KEMRI/SERU/CGMR-C/084/3454) and Oxford University Tropical Medicine Research Ethics Committee (OXTREC 14–17) gave prior approval for the study.
