: This study received ethical approval from institutional and national ethical review boards, namely - the Joint Research Ethics Committee (JREC) at Parirenyatwa Central Hospital and the University of Zimbabwe Faculty of Medicine and Health Sciences at the University of Zimbabwe (Approval number JREC:06/15), and MRCZ (Approval number MRCZ/A/2051), respectively. Permission to access research protocols was also obtained from the Board of the MRCZ. To ensure confidentiality, study staff were required by MRCZ to sign a confidentiality agreement and to collect data at MRCZ premises using the approved data abstraction form. No researcher or protocol identifiers were collected, and no protocol documents were allowed to be taken outside MRCZ offices.
