Ethics approval and consent to participate
: The research protocolRef. KNH-ERC/A/375to conduct this study in clinical setup was approved by Kenyatta National Hospital/University of Nairobi Ethics Research Committee (KNH/UoN-ERC). The approval period was between 26th November 2013 and 25th November 2014. The protocol required written consent from participants because data was obtained from clinical setting. The consent to participate was obtained from each subject through signing of informed consent forms. For minors under 16 years of age, written consent was obtained from their parents or next of kin in order to participate as passive or active respondents.
