: The study protocol was approved both by the National Bioethics Committee of Iceland (NBC; Reference No. VSNb2010050008) and the Icelandic Data Protection Authority (DPA; Reference No. S5680). According to Icelandic legislation (Act on the Protection of Privacy as regards the Processing of Personal Data, No. 77/2000) neither epidemiologic research nor the use of anonymous data generated from clinical service in scientific research projects require individual consent if the study protocol is approved by NBC and DPA, see further .
