: Not applicable. This study was conducted as part of a service improvement procedure and as such did not require separate ethical approval. The work detailed here was approved by relevant authorities at Public Health England and the North Bristol NHS Trust. Data anonymisation was performed at source, prior to analysis in a manner which conformed to the Information Commissioners Office Anonymisation Code of Practice.
