: All participants agreed with the informed consent that was read out and recorded at the start of the interviews. The procedures of this study were in accordance with the declaration of Helsinki. This study has been approved by the Medical Ethics Committee of the University Medical Center Groningen.
: All clinicians participated on a voluntary basis and were granted confidentiality. They all agreed on the final report of their interview and the publication of this manuscript. All clinicians and quotes are anonymized, therefore no written permission was needed in this case.
