: All methods were carried out in accordance with relevant guidelines and regulations. Based on the guidelines and procedures of the Colorado Multiple Institutional Review Board on Human Subjects Protection the analysis of the NPDS data for this research study does not meet the criteria for human subjects according to the 45 Code of Federal Regulations (CFR) 46.101(b) and no approval of the institutional review board was required. Certain categories of research, exempt research, are not subject to federal regulations and do not require convened Institutional Review Board (IRB) review and approval.
