: This study received ethics approval from North York General Hospital Research Ethics Board (REB ID: NYGH #20–0014). All participating primary care physicians provided written informed consent for the collection and analysis of their electronic medical record data at UTOPIAN; patients rostered to a particular primary care physician could opt-out of providing their data to UTOPIAN if they so chose. This model of consent was approved by REB and is consistent with Ontario's privacy legislation (PHIPA Sect. 44).
